Job Summary:

We are seeking a dedicated Patient Enrollment Specialist to join our team. The Patient Enrollment Specialist will be responsible for managing the enrollment process of patients, providing guidance to potential patients, and ensuring a smooth transition into our chronic care monitoring and remote patient monitoring programs.

Duties:

• Collaborate with various departments in the Doctor’s office to develop enrollment strategies
• Speak with patients to explain the remote patient monitoring and chronic care management programs and what the patient’s responsibility will be
• Assist individuals in completing enrollment forms, building chronic care plans and understanding the programs

Requirements:

• The candidate must be bilingual and be able to fluently speak both English and Spanish
• Coordinate with clinic staff to ensure seamless onboarding of new patients
• Maintain accurate records of enrollment data
• Provide ongoing support to patients throughout the enrollment process
• Medical office operation experience required

Qualifications:

• Sales experience, and/or customer service experience are highly desirable
• Strong project management and program development skills
• Excellent communication skills
• Proficiency with modern technology
