The Role:

The SAIL Program Lead is part of the SAIL Program leadership team. The Program Lead is responsible for a range of direct-care programming and staffing-related tasks for the individuals served in the SAIL Program.

Essential Responsibilities:

Responsible for carrying the SAIL Program cell phone during shift hours; answering/resolving and directing phone calls and/or emails to the appropriate personnel. When in custody of the SAIL program phone, the Program Lead will accept resident calls for assistance and a.) Assist individual as needed, and/or b.) Delegate the task to another Program Staff member (DSP1). to provide assistance. Welcome and direct visitors & contractors as needed with the security of the residents in mind. The Program Lead is responsible for maintaining an accurate census while on shift and delivering the details to the overnight shift. Collaborate with Program Managers and Coordinator to create and oversee the evening SAIL activities, including regular exercise and social engagement opportunities. Medication certification required. Evening lead will administer medications (or established designee) according to protocol established by the SAIL RN. Collaborate with the nurse program as needed. Assist DSP1 staff members with life skill assignments as needed (the evening lead will be responsible for

Evening Program Lead will complete shift report daily and submit to Program Managers. Troubleshoot any issues with potluck and/or report concerns to Camille. Utilize RAMP cards as needed to ensure residents have access to a meal. Training drills as required by supervisor and/or

QA. SAIL

Program Lead will access the Program Manager’s office to retrieve debit cards, following protocol for the cards. SAIL Program Lead will have access to nurse’s office to retrieve medications for those residents who are travelling and/or troubleshooting medication issues after hours. Monitor SAIL program inventory and report needs to managers and/or operations. Print CH REC rosters and put in the 2nd floor office every Friday @ 4pm. Using the PO process, order office supplies (ensuring staff offices have pens, sticky notes, hand sanitizers, masks, etc.). Attend SAIL Management/staff/agency meetings as required.

Minimum Education Requirements:

Experience and knowledge working with individuals with disabilities preferable

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Well organized, with flexibility and ability to manage multiple tasks at once Strong attention to detail Must be reliable, flexible, professional and receptive to staff Ability to take direction, take initiative and work within a team. Valid Connecticut driver’s license Able to adapt to flexible nature of the position and positive work attitude Excellent effective judgment, decision-making and communication skills. Computer literacy includes Microsoft Outlook or related programs. Obtain/maintain in-service requirements as mandated by DDS and Chapel Haven Schleifer Center, Inc. Ability to stand, sit, walk or climb stairs for periods of time as needed Ability to obtain & maintain DDS medication administration certification Do not meet every single requirement?
