Resident Care Coordinator 4.0 4.0 out of 5 stars 240 Bates Street, Lewiston, ME 04240 Are you interested in supporting residents through collaboration, advocacy, and compassion? We are hiring a Resident Care Coordinator to provide supports to clients in affordable housing. This position provides a full range of care coordination services and health promotion and travels to meet with clients/families/teams regularly and is full-time 40 hours per week. This position is based out of Lewiston, with travel in service area required. •$5,000.00 Sign On Bonus•
Essential Duties :
Provides direct service through face-to-face, phone calls, or collateral care coordination contact. Responsible for overall management of the plan of care, including drafting the plan of care, completing quarterly reviews, and summarizing pross toward goals. Requires documentation in the Electronic Health Records database. Ensure that participants are connected to primary care services, obtaining necessary consent and releases, and contributes to information sharing as necessary.

Mileage Reimbursement Desired Qualifications:
Associate’s Degree in Human Services or related field required or college coursework equivilent required Bachelor’s Degree in Human Services or related field preferred Holds a valid MHRT-C or eligible for provisional certification At least one year experience for Care Coordination/Case Management is a plus
