Care Coordinator Supervisor Community Development for Asian Americans New Hyde Park, NY 11042 Job Summary We are seeking a dedicated and compassionate Care Coordinator Supervisor to join our team. The Care Coordinator Supervisor oversees and directs a team of care coordinators, ensuring they provide quality, compliant care to individuals. They manage caseloads, provide training and supervision, and ensure adherence to regulatory standards and best practices. Duties Supervising, training, and developing care coordinators, including providing guidance, coaching, and feedback Assigning and managing care coordinator caseloads to ensure equitable distribution and efficient service delivery. Monitoring and evaluating care coordinator performance, including reviewing case notes, care plans, and incident reports Ensuring compliance with regulatory requirements, including LHH (Lead Health Home), CMA (Community Management Agency), and DOH (Department of Health) standards Requirements 5-10 years experience in leadership Understanding of care coordination processes, best practices, and relevant regulations Knowledge of ACBS best practices Knowledge of CORE services Health homecare services experience Ability to motivate, guide, and develop a team.
