Coordinate and administers the service plan to meet health needs of the residents and promote wellness. Supervisors and delegates care to LPN’s CMA CNA and HHA’s & within scope of practice. Essential Duties and Responsibilities include, but are not limited to the following: Assist or act on behalf of the Executive Director as directed. Supervise, manage, direct, and counsel all Health Service employees. Provide appropriate training and direction for all Health Service employees. Oversee departmental staff assignment of tasks and duties. Meet with every new resident and complete a basic health assessment within 72 hours of their moving into the facility. Complete a comprehensive assessment and service plan for each Resident per company and state required frequency. Assist in and verify that all assessments and documentation are current and accurate. Maintains confidentiality and HIPAA regulations in accordance with facility and state/ federal policies with staff, residents, and Company information. Administer medications and perform treatments per doctor’s orders. Coordinate the assistance plan, activate resources through home health, and communicate with the physician regarding resident issues. Provide or coordinate educational opportunities related to health issues of interest to the residents. Implement the self-administered medication management program. Collaborate with pharmacy to ensure accurate medication management program. Assist in completion of required documentation to meet federal and state guidelines as well as Company protocol/ policies. Review all emergency calls for assistance by residents and complete a quality-action review on the treatment and handling of each emergency by staff. Develops a service plan for each resident and ensures follow through and revision as needed. Collaborate and maintain routine communications with families, residents, and the medical community. Monitor written staff documentation, including daily documentation sheets, electronic records, communication book and self-administered medication sheets for assurance and completeness. Participate in meetings to discuss needs and requirements of each resident and takes appropriate action as needed and directed to promote quality resident care. Provide staff education for all departments- including on-going instruction and guidance as related to the Health Service Department. Assuring those residents have physical, emotional, social and spiritual needs met. Work is of high quality and done in a pleasant manner. Carry out assignments as directed, when directed. Complete all required competencies. Follow scope of practice. Ensure resident safety. Audit all aspects of the health services department to verify evidence-based practice is followed. Inventory and procurement of supplies as needed for resident care. Ensure excellent customer service by all staff. Model a positive and professional attitude and environment for all staff. Maintain license in good standing. Answer resident call bell system. Maintain the budgetary requirements as directed. Work schedule as assigned. Assist in covering shifts when required. Maintain a clean and safe environment. Track and counsel employees as appropriate related to attendance and performance. Ensure staff are following procedures and policies and are being productive and efficient.

Knowledge/Expectations:

Employee/Department Management, including hiring, proper disciplinary documentation, termination of staff employment as necessary and scheduling of department employees. Nursing standards of practice for older adults. Resident assessment tool. Assisted Living state regulations in

GA Qualifications:

Nursing License in GA. Experience in working with older adults. Good assessment skills. Good communication skills.

$80,000.00 – $85,000.00 per year
