Organizational Overview :

Optimum Choice Services, Inc. is seeking an experienced clinicians to join our team. Optimum Choice is a multifaceted clinical service provider operating under the auspices of the New York State Department of Health Traumatic Brain Injury Medicaid Waiver program serving adults who have sustained a traumatic brain injury (TBI) . We are operating in the New York Metropolitan area, Lower Hudson Valley and Long Island regions. Our goal is to provide individualized services to participants engaged in the TBI Program.

Job Summary:

We are currently seeking full time/part time and per diem clinicians for positions within the Positive Behavioral Intervention program .This is an individualized clinical service designed to reduce the frequency and impact of behavioral disturbances which, without intervention, may jeopardize community living. The PBIS staff assist the waiver participant effectively manage residual, emotional difficulties associated with adjustment following a traumatic brain injury. Therefore, PBIS strives to provide support through structured therapeutic interventions designed to mitigate disruptive behavioral patterns. This service is offered in either the provider’s professional environment or in the participant’s home.

Role & Essential Functions :

Provide education and direct behavioral intervention to individuals with a variety of disabilities and their natural supports as specified. Complete behavioral assessments in collaboration with the participant, natural supports and other staff as required. Develop behavioral treatment plans based upon data obtained during the initial assessment process. Treatment plans will be designed utilizing a holistic and person-centered approach which describes successive levels of intervention. Treatment plans both outline mechanisms for intervention but also serve as implementation guides for non-clinical supports. Therefore, treatment plans must be constructed utilizing language that is concrete and relatable. Provide direct service delivery as outlined in the respective Service Plan. Implement the developed treatment plan with the participant during routine sessions. Provide training related to treatment plan interventions with other providers and with natural supports. Proactively engage in ongoing monitoring of behavioral needs and modify existing treatment plans in collaboration with the PBIS Supervisor/Director. Complete required session notes for all encounters through use of the designated electronic medical record system. Complete required documentation for clinical interactions including but not limited to the following: behavioral assessments, behavioral treatment plans, individualized service reports, progress notes and clinical evaluations.

Qualifications:

Persons employed as a Behavioral Specialist must be a/an: 1. Individual with a Bachelor’s degree in psychology; 2. Registered Professional Nurse (licensed by the NYS Education Department); 3. Licensed Occupational Therapist (licensed by the NYS Education Department); 4. Licensed Physical Therapist (licensed by the NYS Education Department); 5. Certified Behavior Analyst Assistant (Certified by the NYS Education Department); 6. Individual with a Bachelor’s degree and a Certified TBI Specialist; 7. A Certified Rehabilitation Counselor (certified by the Commission on Rehabilitation Counselor Certification); 8. Individual with a Bachelor’s degree and certified in Applied Behavioral Analysis; 9. Individual with a Bachelor’s degree and a Certified Dementia Practitioner; 10. Individual with a Bachelor’s degree and a CARES Dementia Specialist; AND Must have two years of experience developing and implementing intensive behavioral treatment plans.

$35.00 – $40.00 per hour
