Director of Referral Management and Care Coordination 3.9 3.9 out of 5 stars 3131 S Main St, Moultrie, GA 31768 Overall supervision and direction of referral management and care coordination. Participate in various hospital committees. Monitor market share and collaborate with marketing to adjust strategy as needed. Maintains close contact with the public, public officials, media, businesses, medical staff, etc. Job Duties Inlude, but are not limited to: Work with the marketing department to develop accurate and engaging materials for service lines. Develop and implement marketing plans for external services, physicians as needed. Develop product lines as directed by leadership Assist in physician recruitment efforts as requested. Prepares annual budget for department Assist in developing the annual strategic plan regarding marketing and advertising, increasing market share, adding new service lines. May serve as a point of contact for generating and coordination of new patient referrals, as needed Attending hospital patient care and discharge readiness rounds. Meet prospective patients and families that can potentially benefit from service of the health system. Work as a member of the various interdisciplinary teams for patient care coordination Must have a commitment to and understanding of the external service and other heath system services. Participate in community relations. Continuously conducts market assessments and develops a comprehensive marketing plan designed to meet budgetary volume projections. Monitors allocation of resources according to budgetary limitations. Establishes and maintains positive working relationships with current and potential referral and payer sources. Maintain a database of medical contacts and community resources. Knowledgeable about industry trends and legislative/regulatory issues. Communicate information with staff and community contacts. Develop and implement a marketing strategy that reflects service opportunities, competition, and potential market area. Develop and implement special events and presentations targeted at community education and establishing and maintaining referral sources. Monitor response times to inquiries and provide appropriate follow-up. Ensures admission screening of potential residents / patients. Ensure company processes are followed to ensure a timely clinical and financial approval process. Ensures consistent and effective process for coordination of admissions with appropriate departments and staff. Contacts local community agencies to generate a positive image and encourage word-of-mouth referral activities. Ensures adequate time is spent outside the building on sales activity. Patient review and exploration of insurance eligibility Typing correspondence and other assigned material efficiently and accurately Maintains confidentiality of all hospital & staff information, and patient information per HIPAA guidelines Maintains proper inventory of appropriate and required office and medical supplies Maintains staff payroll reporting, scheduling, and PTO. Maintain expense reports for staff as needed Ensure that staff performance appraisals are completed on time Reconciling and approving invoices for accounts payable Maintain and support Colquitt Regional Health System initiatives Leadership Skills Ability to build professional relationships with staff, physicians, peers, patients and families. Ability to communicate effectively both in normal day-to-day operations as well as in challenging situations. Remains calm under pressure and always maintains professional appearance. Honesty and integrity in all interactions. Critical thinking and reasoning, computer skills, financial/business knowledge, leadership, and resource management are expected capabilities. Leaders are expected to average 60% of their time working on day-to-day operations and 40% on management and administrative responsibilities. Expected to design and execute outreach strategies, develop partnerships with local organizations, as well as plan and execute events.
