Job Description:

A Brief Overview To manage and assist in the development, implementation, utilization and maintenance of the day-to-day operations of the Home Health Care and Hospice Departments at Great River Health Sytems. Works closely with the home health and hospice team in the management of operations and plays an integral role in coordination of functions for home health and hospice services, including but not limited to—hiring, suspending, laying off, recalling, promoting, discharging, assigning work, and rewarding. What you will do Employee Performance Management

  • Perform direct supervision of staff, to include clinical reviews.

Plan and prepare employee work schedules. Ensure that staff applies primary knowledge while performing all aspects of assigned tasks. Monitor and provide feedback to staff who develop, implement, and evaluate a plan of care for patient. Participate in training and orientation of new staff; provides ongoing education. Evaluate employee performance and personnel problems, and to discuss company policies. Forecast necessary levels of staffing and stock at different times to facilitate effective scheduling and ordering. Conducts performance reviews and provides input in aligned job families. Identifies and leads opportunities to enhance staff productivity and operational processes, tools and services. Departmental Oversight

  • Oversee the workflow of the department and communicate standards.

Responsible for a job family within the health system and provides guidance using technical expertise. Makes independent decisions, interprets and executes policies and regularly makes recommendations. Works with established policies/procedures to solve moderately complex matters in aligned process. Coordinate activities with other departments to ensure that services are provided in an efficient and timely manner. Implement and maintain production and quality standards for the staff. Work with Clinical management staff to ensure that thorough and sound clinical assessments of presenting symptoms are conducted. Respond to complaints in a timely manner. Inventory stock to ensure that supplies and equipment are available in adequate amounts. Issue supplies and equipment and cars to workers. Maintain required records of work hours, budgets, payrolls, and other information. Perform or assist with departmental operation duties as necessary. Provides input to departmental leaders for policies/procedures and strategies to improve operational efficiencies. Receives tactical and strategic direction to meet established direction to meet established goals and operational priorities. Recommend changes that could improve service and increase operational efficiency. Screen job applicants, interview and hire new employees. Safety and Compliance

  • Ensure that documentation is completed according to department policies and procedures.

Investigate complaints about service and equipment and take corrective action. Inspect work performed to ensure that it meets specifications and established standards. Analyze need for new equipment, supplies, or furnishings, and request from department director. Prepare for and participate in state survey and audits. Qualifications Associate’s degree (AA) In Nursing (ADN) Required Bachelor’s degree in arts/sciences (BA/BS) In Nursing (BSN) Preferred 4-6 years Health care experience within the last five years Preferred Minimum of 5 years’ experience in nursing practice, prefer leadership experience Required 1-3 years’ Experience with Microsoft Suite (Word, Excel, PowerPoint, Outlook, and Teams) Preferred Knowledge of machines and tools, including their designs, uses, repair, and maintenance. Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources. Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders. Knowledge of relevant equipment, policies, procedures, and strategies to promote effective local, state, or national security operations for the protection of people, data, property, and institutions. Understanding written sentences and paragraphs in work related documents. Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Teaching others how to do something. Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action. Understanding the implications of new information for both current and future problem-solving and decision-making. The ability to communicate information and ideas in speaking so others will understand. The ability to tell when something is wrong or is likely to go wrong. Home Health Manager

  • will have additional requirements for knowledge, understanding, and billing of home health including CMS conditions of participation. Hospice Manager
  • will have additional requirements for knowledge, understanding, and billing of hospice including CMS conditions of participation. Travel may be required for interdisciplinary or case care planning conferences.
  • Expected to have knowledge and understanding of both services to provide leadership coverage and support.
  • Additional travel may be required for supervision, training and meetings.
