PM Job Description Purpose:
To work at the discretion of the Associate Director and Assistant Director of Primary Care in Ambulatory Care supporting ambulatory practices as but not limited to Population Health Director, Ambulatory care and partner with care team to provide additional support to manage care team in-baskets, residency clinic and administrative needs as well as lead ambulatory performance improvement projects as assigned by leadership.
Specific Duties and Responsibilities:
The Coordinating Manager Level B is charged with and responsible for: Assisting with projects as assigned by Ambulatory Care Leadership.
Leading efforts to monitor, direct, and respond to in-basket messages for the ambulatory care practice.
Facilitating effective communication between providers, nurses, and front desk staff to ensure smooth operations.
Coordinating patient letters, forms, faxes, and scheduling patient appointments as needed.
Providing population health care coordination in partnership with the ambulatory care team to ensure comprehensive patient care.
Reviewing weekly population health reports, addressing care coordination gaps, and following up with the care team.
Provides direct support to the residency clinic.
Serving as a liaison for patients requiring additional resources (e.g., Social Workers, Community Health Workers) to overcome social or systemic barriers to care.
Assisting ambulatory care leadership with initiatives such as PCMH (Patient-Centered Medical Home), ACO (Accountable Care Organizations), and Collaborative Care programs.
Planning, developing, and conducting training programs to ensure staff proficiency and adherence to best practices.
Ensuring that appropriate registration and financial screening processes are properly followed to optimize patient flow and revenue.
Consulting with the Assistant Director to improve operational processes and workflow.
Running daily reports to track clinic statistics and inform decision-making.
Providing administrative coverage as needed within the Ambulatory Care Administration office.
Providing coverage at Ambulatory Care front desks when necessary.
Serving as a backup for tracking and managing time off (vacation/sick) for
PAGNY/H +H
medical and medical specialty employees in the absence of the Coordinating Manager.
Ensuring compliance with policies, procedures, and departmental standard work protocols.
Performing other duties as assigned.
Minimum Qualifications
Master’s degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling; and one (1) year of full-time experience in medically-oriented health care and medical support systems environment; or
Bachelor’s degree from an accredited college or university in disciplines listed in “1” above; and two(2)years of full-time experience in medically-oriented health care and medical support systems environment; or A satisfactory equivalent combination of education, training and/or experience, in which each year of relevant training and/or experience can be substituted for 30 semester credits from an accredited college or university. However, all applicants must at least have a Bachelor’s degree or its educational equivalent.
Department Preferences
Competence in Word, Excel, and EPIC Cadence functions is necessary
Strong organizational, communication and time management skills required
Administrative experience or an equivalent combination of training, educational background and experience in related fields and educational disciplines
Overall knowledge of the fundamentals of hospital organization, administration, standard regulations and laws applicable to hospital operations
Multi-tasker, detail oriented with excellent follow-up skills and a proven ability to exercise initiative and judgment
Flexibility for weekend and/or evening coverage
