$94,000-$150,000/year Job details Job Description Coordinates the transplant process for transplant recipient/donor patients. Maintains clinical responsibility involving QA reporting and data maintenance. Provides community education and outreach programs. Job Responsibility Coordinates the organ transplant process for prospective candidates; reviews referrals and medical records of candidates; schedules evaluation testing for transplant candidates; assesses the results of evaluation data with transplant team; coordinates the organ transplant process for scheduled pre-transplant patients; coordinates the pre-organ transplant evaluation process for hospitalized patients.
Maintains medical records and ensures that patient files are complete, updates database as necessary; coordinates the plan of care for organ transplant patients; collaborates with transplant team in the planning and implementation of a comprehensive pre/post operative plan of care; provides support and consultation to co-workers and peers in administering patient care.
Ensures all necessary tests are completed including, but not limited to, scheduling appointments for patients between various departments within the Hospital and ensures all relevant medical information is available prior to scheduled appointments; following-up on the results of all pending tests.
Maintains pre-transplant patient files and ensures that patient files are complete; reviews baseline pre-transplant workups, laboratory and diagnostic for scheduled organ transplant patients; evaluates final transplant workups with the transplant team; educates patients, family members about the transplant process; answers appropriate questions as necessary.
Performs appropriate follow-up of post-operative transplant patients; ensures patient assessment has been completed and documented; identifies actual/potential patient problems.
Collaborates with post-operative care members and transplant surgeon; ensures appropriate post-operative plan of care; serves as a liaison between the patient and health care members.
Relates patient concerns and confers with transplant team; makes referrals to appropriate resources as necessary; serves as a liaison for transplant physicians, co-workers, referring physicians, patients and their families, staff nurses and external patient care organizations.
Provides information and counseling to transplant patients and significant others; provides basic information and education about the transplant process to patients, families and community; reinforces information presented by the transplant surgeon.
Counsels patients, answering questions as appropriate; cares for and manages the care of patients with End-Stage Renal Disease (ESRD) awaiting transplant; completion of all required data as established by the Organ Procurement Transplant Network.
Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions. Job Qualification Nursing Diploma required, or equivalent combination of education and related experience. Must obtain a Bachelor of Science in Nursing degree within 5 years of employment date.
Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
5-7 years of technical experience and 2+ years of leadership / management experience, preferred.
OR Master of Science required, or equivalent combination of education and related experience.
Current License to practice as a Physician Assistant in New York State required, plus specialized certifications as needed.
