Full Job Description Responsibilities:

• Provide direct patient care in accordance with the care plan established by the Nurse or other healthcare professional
• Assist patients with activities of daily living, including bathing, dressing, grooming, and toileting
• Prepare and serve meals to patients according to their dietary needs and restrictions
• Assist with medications as prescribed by the healthcare team
• Monitor and record vital signs, such as blood pressure, temperature, and pulse
• Assist with light housekeeping tasks, including cleaning and organizing patient’s living space
• Maintain patient confidentiality and adhere to HIPAA regulations
• Lift and transfer patients safely using proper body mechanics and assistive devices
• Provide emotional support and companionship to patients

Requirements:

• Previous experience in a nursing home or home care setting is preferred
• Ability to follow instructions and work independently with minimal supervision
• Knowledge of basic medical terminology and procedures
• Strong communication skills, both verbal and written
• Ability to work with individuals with developmental disabilities or special needs is a plus
• Familiarity with maintaining medical records accurately and efficiently
