Full Job Description Broad Function:
The services provided by this staff, will be provided to individuals supported thru the Innovations Waiver (CAP-IDD) who choose to live in their own home and receive supports while residing in their own homes. Services and supports are necessary to assist the person in achieving and maintaining individual goals. The service includes direct assistance as needed with activities of daily living, household chores essential to the health and safety needs of the person supported, budget management, attending appointments and interpersonal and social skill building to enable the person to live in their own home in the community.
Major Responsibilities:
Paradigm, Inc. holds its staff accountable for assiting people supported to achieve personal outcomes and to have the best life possible. Staff are expected to support the person by: 1. Assiting them to realize their goals and attain personal outcomes 2. Assisting them to develop and maintain a social support network 3. Assisting them to maintain or re-establish their family connections 4. Assisiting them with maintaining their home/environment. 5. Assiting them with maintaining their overall helth and well being. 6. Assisting them with with utilizing available resources within their community 7. Assisting them with directing their services and being able to make informed choices in all aspects of their life. 8. Assisting them to live and work in a safe environment, free from abuse, neglect, and exploitation. Expectations also include the staff advocating for people with disabilities and to teach members of the community to treat people we support with respect and dignity by modeling positive attitudes and respectful communiication and interactions.
Duties and responsibilities:
Primary responsibiltities include but is not limited to: Practice of skills or direct assistance related to activities of daily living (personal grooming/care, cleanliness, household chores, meal preparation, etc.) Assistance with personal budget and budget management Assistance with attending appointments which includes arranging or providing transportation 4054 S. Memorial Dr., Suite K Winterville, NC 28590 (252) 561-8112 Office (252) 561-7455 Fax 2 Training on interpersonal and social skills training Training activities, supervision, and assistance provided to allow the person to particpate in home life and communtiy activities Monitoring health status and physical condition Assistance with transferring, ambulation, and use of special mobility devices as needed Providing transportation or ensuring arrangement of transportation as needed to ensure involvment in the community •Other duties are included and may not be necessarily included in the list above. These additonal duties are dependent upon the needs of the person supported to ensure their health and safety
Qualifications and Skills:
Qualified in the customized needs of the person/s as decribed in the ISP Possess knowledge of the population that is being served Must meet client specific competencies as identified in the ISP Must meet requirements for paraprofessionals as outlined in 10A

Must be able to communicate orally and written Provide support to the individual that is being served in a respectful and non-demeaning manner
Requirements and Trainings:
Have a high school diploma or equivalancy (GED) Current and up to date First Aid and CPR training Have an acceptabel criminal background check that presents no helath and safety risk to the person or persons being supported by this service Possess a valid NC driver’s license or other valid driver’s license, a safe driving record and and an accpetable level of automobile liability insurance No findings in the NC Helthcare Abuse Registry Must be at least 18 years of age Physical and annual TB skin test Completion of provider orientation Certifications in HIPAA, Participant Rights, Confidentiality, OSHA/Bloodborne Pathogens, Medication Administration and other trainings that are specific to meet the needs of the person served i.e., Crisis Response, Specific Diagnoses Trainings in
