Home Health Aide Coordinator 4.4 4.4 out of 5 stars West Hills, NY 11743 Overview We are seeking compassionate and a dedicated Coordinator to provide essential care and support to individuals in their homes. This role is vital in assisting clients with coordinating care, caregivers, family needs and MLTC’s authorizations. Ensuring the client’s comfort, safety, and well-being. The ideal candidate will have experience working with individuals with disabilities, dementia care, caregivers and a strong commitment to enhancing the quality of life for our clients. Responsibilities Coordinate clients, their families, Insurance companies and caregivers. Provide coordination for several Clients. Help clients with finding the proper caregiver for their needs. Maintain accurate medical records and documentation of client care and caregiver compliance. Coordinate basic medical care as needed. Support clients with memory care techniques for those experiencing dementia or cognitive impairments. Coordinate with healthcare professionals and caregivers to develop and implement individualized care plans. Help coordinate the cleanliness and safety of the client’s living environment. Requirements Previous experience in a home health setting is preferred. Familiarity with systems related to coordination of care for individuals with disabilities or developmental disabilities is a plus. Knowledge of dementia care practices is highly desirable. Strong communication skills and the ability to build rapport with clients and their families. Knowledge of MLTCs, DSS, HHA, NHTD and state caregiver regulations is needed. Compassionate nature with a genuine desire to help others improve their quality of life. Must be able to pass background checks as required by state regulations.
