Position Overview:
The Individual Supports Specialist provides one-on-one assistance to individuals in their homes and community settings. This role focuses on helping each person achieve their unique goals related to daily living, personal growth, and overall well-being. Key Responsibilities:
Provide individualized support to help clients with activities of daily living (e.g., hygiene, meals, dressing, medication reminders). Assist with household tasks such as cleaning, laundry, and organizing. Support clients in developing life skills, communication, and socialization. Monitor and document progress toward personal goals and health outcomes. Encourage independence and self-advocacy. Follow support plans and ensure the safety and dignity of every client. Work closely with families, case managers, and healthcare providers to deliver consistent care.
Qualifications:
High school diploma or GED required; relevant training or coursework in human services is a plus. Previous experience providing direct support to individuals with disabilities strongly preferred. Valid driver’s license and reliable transportation (if community access is part of the service). CPR/First Aid certification (or willingness to obtain). Strong communication and interpersonal skills.
