Position Summary:
The Healthcare Coordinator has an imperative role in compliance with the Agency by maintaining accurate and updated client records and charts. They are responsible for completing the intake of new clients/referrals and act as a liaison between clients, client families, and health care professionals. They perform administrative duties such as assisting with the coordination of care for clients and team members and offering support to Clinical Department. This role is ideal for a licensed practical nurse with strong organizational and administrative skills who thrives in a dynamic, field-based care environment.
Key Responsibilities:
Maintain and track medical orders and client records Serve as a liaison with physicians, hospitals, and insurance providers Ensure compliance through audits, data collection, and QAPI participation Support care coordination from admission to discharge Assist with clinical staff training and orientation Improve internal workflows and systems Provide administrative support including calls, data entry, and document handling
Qualifications:
Active LPN license in the state of FL. 3+ years of nursing experience, preferably in home health or community-based care 1+ year of administrative experience in a healthcare setting Solid understanding of home health compliance standards and documentation practices Exceptional interpersonal, organizational, and leadership skills Proficient in EMR systems (e.g., WellSky, Axxess, Homecare Homebase, or similar) Bilingual preferred, but not required.
