  • The Healthcare Coordinator is responsible for successfullysupporting patients with stable chronic health conditions tonavigate the healthcare system. The Healthcare Coordinator assistsin developing patient empowerment by acting as an educator,resource, and advocate for patients and their families to ensure amaximum quality of life.
  • The Healthcare Coordinator interacts andcollaborates with multidisciplinary care teams, to includephysicians, nurses, pharmacists, laboratory technologists, socialworkers, and other educators. The Healthcare Coordinator works in aless structured, self-directed environment and performs all nursingduties within the scope of a LVN/LPN license of the applicablestate board of nursing.

Primary Responsibilities:

Works with the providers and clinic staff to identify patientswith chronic disease diagnoses. Supports longitudinal care of the patient with stable chroniccare conditions by: performing focused assessment of chronic careconditions performing medication reconciliation conducting Motivational Interviewing and Self-Management Goalsetting providing patient education using standardized, approvededucational materials creating referrals to appropriate agencies and resources Supports transition of the patient with stable chronic careconditions from inpatient to outpatient setting, by: performing focused assessment of transitional needs performing medication reconciliation reviewing contingency plan providing patient education using standardized, approvededucational materials assisting with post discharge needs such as prescriptions,transportation, Durable Medical Equipment (DME), appointments Notifies providers and/or supervising RN of changes topatient’s health condition Performs accurate and timely documentation in the electronicmedical record Participates in daily huddles and Patient Care Coordination(PCC) meetings Prepares accurate and timely reports, as required, for weeklymeetings Maintains continued competence in nursing practice andknowledge of current evidence based practices May perform clinical tasks within their scope ofpractice Performs all other related duties as assigned

RequiredQualifications:

Current unrestricted Texas LVN license or compactlicense Current BLS certification 2+ years of experience in a physician’s office, clinical orhospital setting Knowledge of chronic diseases, especially COPD/asthma,diabetes, CHF and IHD Proficient computer skills to work efficiently with electronicmedical records This position requires Tuberculosis screening as well as proofof immunity to Measles, Mumps, Rubella, Varicella, Tetanus,Diphtheria, and Pertussis through lab confirmation of immunity,documented evidence of vaccination, or a doctor’s diagnosis ofdisease

  • The hourly range for this role is $19.

86 to $38.85 per hour basedon full-time employment. Pay is based on several factors includingbut not limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
