Jackson, Platte, Clay, CassCounties in Missouri The role you’ll contribute: This position isaccountable for providing professional nursing activities asidentified and planned by the R.N. Case Manager in the Patient CarePlan to maximize function within the home restoring health orassisting a peaceful death. The L.P.N. is supervised in the home bythe R.N. as often as necessary, but at least every sixty days. Thevalue you’ll bring to the team: Implement individualized patientcare as initiated by the R.N The expertise and experiences you’llneed to succeed: Minimum qualifications: Graduate of an approvedschool of nursing At least one (1) year of nursing experience in ahospital setting or community setting involving direct nursing careCurrent license to practice as a professional nurse in the State ofKansas and Missouri. Active American Heart Association BLS ValidDrivers license Preferred qualifications: Previous home healthexperience.
