Provides skilled nursing care to patients in their home under the supervision of a Registered Nurse. Provides service in accordance with the Hospice’s philosophy, standards and policies. Demonstrates Competency in the

Following Areas:

Provides hospice nursing care and ongoing support in the home as ordered on the plan of treatment and nursing care plan. Functions as a team member demonstrating collaboration with, and responsibility to patient, RN Case Manager, physician and interdisciplinary group. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Reassesses pain and notifies the registered nurse when pain is not manageable on the current medications. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Performs patient care responsibilities considering needs to the standard of care for patient’s age. Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality. Adheres to all Hospice, payroll, billing and documentation policies and procedures. Participates in Hospice’s other mandated activities, as requested by the Clinical Nurse Manager including inservices and QAPI committee. Flexible, organized, and posses the ability to exercise sound judgment. Understands Hospice philosophy and issues of death and dying. Completes all other tasks as requested. Attends all meetings as required #
