Live Long Well Care is seeking an experienced Home Care LPN towork two to three shifts per week. Hours are 8:00 am
- 4:30 pm.
Office is conveniently located just off of I-40 and Stratford Road.
This position is responsible for providing some direct care andcompliance support for nursing as well as possibly assisting withsome outreach directly to prospects, hospitals, physician offices,rehab clinics, senior organizations. We are private pay (notMedicare or Medicaid certified) and provide private duty skilledand non-skilled services for the senior/elderly population. Thisposition reports to the CSM (Client Services Manager).
POSITIONSUMMARY
Provides hands on resident care, proper and timelyadministration of medications and treatments, accurate verbal andwritten reports and rapid response to any/all emergency situations.
Level I
- Basic competencies and knowledge of nursing: ClientExamination and Assessments.
ESSENTIAL FUNCTIONS
Performsprofessional and restorative nursing services which include, butnot limited to, resident assessments, recording of vital signs,administering medications and treatments. Keeps timelydocumentation of written reports and verbal reports ensuring thatquality care is provided. Interacts with the resident’s physician,pharmacist and medical personnel as needed. Following theguidelines as outlined in the Resident Care Plan; assists residentswith activities of daily living including bathing, dressing,eating, toileting, hygiene, and mobility. Assists with in-serviceand on the job training and mentoring of Team Members. Participatesin and attends all required in-service training and educationprograms as scheduled. Other duties as assigned.

Full Physical Demands, Work Environment, andExpected Hours of Work:
The physical demands described here arerepresentative of those that must be met by an employee tosuccessfully perform the essential functions of this job. Thisposition is very active and requires frequent standing, walking,bending, kneeling, stooping, and climbing. This position will berequired to lift or carry weight up to 50 lbs. and ability to pushup to 250 pounds independently. The individual must use proper bodymechanics to assist residents in their daily living. This positionregularly requires long hours and frequent night and weekend work.
TRAVEL:
No regular travel is expected although occasional travelmay be required for training sessions, continuing educationopportunities, emergency situations, and other company functions.
EDUCATION AND EXPERIENCE REQUIREMENTS
Current state licensure as aLicensed Practical Nurse or Licensed Vocational Nurse in goodstanding. Certification in CPR, AED, and First Aid High schooldiploma or GED Four-year college or university degree desired. 1-2years’ experience in a similar healthcare position preferred.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Ability to read, write,speak, and understand English fluently. Ability to meet or exceedthe company’s attendance and punctuality standards. Ability to useElectronic Records and miscellaneous software and office equipment.
Ability to understand and follow directions as given. Ability towork with minimal supervision.
