We are seeking an experienced and mission-minded Resident Care Coordinator to oversee clinical services and ensure our residents receive the highest standard of care.
Position Summary:
The Resident Care Coordinator (RCC) is responsible for managing all clinical aspects of resident care. This leadership role includes training and supervising Medication Technicians, Personal Care Assistants, conducting medication cart audits, overseeing PCS (Personal Care Services) documentation and QI reports, and ensuring compliance with all applicable state and federal regulations. The RCC plays a key role in supporting the overall health and wellness of our residents and leading the care team with excellence and integrity. ONLY LPN or RN should apply.
Key Responsibilities:
Provide daily clinical oversight of resident care, including assessments, service plans, and follow-up care Train, mentor, and supervise Med Techs to ensure proper medication administration and best clinical practices Conduct regular medication cart audits to ensure accuracy, cleanliness, and compliance Complete and monitor QI reports related to PCS services, ensuring documentation is complete, accurate, and submitted on time Oversee the Medication Administration Record (MAR) system and interface with Southern Pharmacy to ensure timely and correct medication delivery Collaborate with the Memory Care Coordinator and provide leadership for the overall care team Ensure all medication administration, clinical procedures, and documentation comply with state and federal guidelines Perform clinical assessments for new admissions and hospital readmission evaluations Monitor and improve the quality of resident care through direct observation, data review, and staff support Serve as a liaison with physicians, hospice, and other external healthcare partners Participate in care plan meetings and ensure individualized service plans reflect each resident’s evolving needs Promote a positive team environment that upholds the mission, values, and standards of
Piedmont Christian Home Qualifications:
Licensed Practical Nurse (LPN) or Registered Nurse (RN) At least 2 years of experience in assisted living or long-term care required Strong understanding of PCS services and clinical documentation requirements Prior leadership or supervisory experience in a healthcare setting Excellent attention to detail, communication, and organizational skills Proficiency with medication records, audits, and regulatory compliance Ability to respond promptly and professionally to emergencies and resident care needs Compassionate, team-oriented, and mission-aligned
