SUMMARY:

As aLicensed Practical Nurse, you will provide professional patientcare to long-term and post-acute care residents. You will directand monitor the care provided by CNAs and consistently ensureproper documentation and recording is taking place. Your ability tolead is crucial in both the success of the CNAs and the care of thepatients. Attached to Clifton Springs Hospital, Clifton SpringsNursing Home offers a uniquely holistic approach to long-term care.

Skilled nursing care is located adjacent to Spa Springs, wherepatients can access massage therapy, acupuncture and similartreatments. Clifton Springs Nursing Home is a 108-bed facility.

RESPONSIBILITIES

• Responsible for timely, accurate medicationadministration of assigned team and concurrent requireddocumentation.
• Performs treatments and documents progress of sameas per physician orders and facility protocol.
• Assures concise,pertinent and complete resident care documentation usingcomputerized medical record process. Assures the initiation ofappropriate care pathway tools.
• Responsible for receiving/givingrelevant reports, both written and verbal from/to appropriate staffas per departmental protocols.
• Assures proper monitoring ofresidents and relevant condition status are reported to chargenurse/supervisor and/or physicians in a timely manner.
• Completespre-reviews of medication administration records as per facilityprotocols.
• Completes required MDS documentation and all relatedobservations in a timely manner.
• Supports CNA team throughassisting in direct ADL care of residents.

REQUIRED QUALIFICATIONS

• Graduate from an accredited School of Practical Nursing.
• Current licensure in State of New York.

PREFERRED QUALIFICATIONS

• One year experience preferred.
