Patient Care Telephone Triage Nurse

out of 5 stars Lakewood, NJ 08701 CHEMED (The Center for Health Education Medicine and Dentistry) is seeking a compassionate and experienced Patient Care Telephone Triage Nurse, LPN or RN to join our team. This is an exciting opportunity for a nurse with a background in Pediatrics and Primary Care who thrives in a fast-paced, patient-centered environment. This job is in a call center!
Schedule:
Monday-Friday 9am-5pm. Every other Sunday you will work a shift and have a weekday off. If you’re a nurse with excellent communication skills and a passion for helping others, we want to hear from you! This opportunity as a Patient Care Telephone Triage Nurse will not last!
What You’ll Do:
Assess Patient Needs:
Conduct phone assessments to determine the appropriate level of care.
Provide Medical Guidance:
Offer clear, protocol-based advice and education on self-care and next steps.
Triage Effectively:
Identify and escalate urgent cases to ensure timely care. Document with
Precision:
Maintain accurate and detailed electronic health records.
Collaborate:
Work closely with our physicians and care teams for seamless patient support.
Deliver Exceptional Service:
Show empathy, patience, and professionalism with every patient interaction.
What We’re Looking For:
LPN or RN License in New Jersey (active and unrestricted) Clinical Experience in Primary Care and/or Pediatrics Outstanding Communication and critical thinking skills Comfort with Technology, including electronic health records (EMR) Team-Oriented mindset with a caring, professional attitude Bilingual is always a plus!
