Conduct thorough patient assessments to monitor health status and implement appropriate care plans. Administer medications and treatments as prescribed, including IV infusions and tube feeding. Maintain accurate and confidential patient records in EHR systems while adhering to HIPAA regulations. Support patients and families emotionally, offering guidance and education about hospice services and end-of-life care.
