LPN – Primary Care Clinic Job Description The LPN will support a primary care clinic by rooming patients, taking vital signs, performing phlebotomy, and following up with patients for procedure/test scheduling or referrals as needed. Responsibilities Record patient’s vital signs and weight. Ensure test results are received and communicated to patients in a timely manner. Document all interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record. Ensure an adequate stock of supplies and well-maintained equipment. Monitor and ensure accuracy of recordings in patient medical records. Maintain all required reports, records, statistics, etc. Maintain confidentiality regarding the practice, its patients, and employees. Use a variety of office and medical equipment, including but not limited to telephone, computer, fax, stethoscope, scales, thermometers, blood pressure cuff, and basic lab equipment. Take calls from hospitals/nursing homes and relay appropriate information to practitioner in a timely manner.
