Collaborate with healthcare team members to develop and implement individualized care plans. Educate patients and their families on health management and wellness strategies. Conduct triage assessments to prioritize patient needs effectively. Participate in case management activities to ensure continuity of care. Support home community care initiatives for patients requiring assistance outside of traditional settings.
