No on-call, no weekends and no holidays required. This rolerequires travel across the entire state of California,approximately 85% of the time. All travel expenses are covered, andthere is likewise a base salary plus multiple incentives.
PrimaryResponsibilities:
- Conduct in-home assessments including anevaluation of past medical history, review of systems, medicationreconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings including point of caretesting (as appropriate)
- Identify diagnoses to be used in caremanagement and active medical management in the furtherance oftreatment
- Formulate a list of current and past medical conditionsusing clinical knowledge and judgment and the findings of yourassessment
- Communicate findings in your assessment that will beused to inform member’s PCP of potential gaps in care
- Identifyurgent and emergent situations and intervene appropriately
- Educate members on topics such as disease process, medication, andcompliance You’ll be rewarded and recognized for your performancein an environment that will challenge you and give you cleardirection on what it takes to succeed in your role as well asprovide development for other roles you may be interested in.
Required Qualifications:
- Active NP license or ability to obtainby start date. Licensure must be unencumbered, free of anyopen/unresolved disciplinary actions including probation orrestrictions against privilege to practice
- Active national NPcertification or the ability to obtain national certification instate of assignment by start date (Active ANCC or AANP nationalcertification in Family, Geriatrics, Adult, Adult-GerontologyPrimary Care, or Adult-Gerontology Acute Care certification)
- Ability to complete physical requirements of the job includinglifting a 30-pound bag in and out of the car, navigating stairs anda variety of dwelling conditions, and sit, stand and kneel toperform physical assessment
- Reliable transportation to completehome visit assessments
Preferred Qualifications:
- 1 years ofclinical experience in their highest level of education, clinicalsetting
- May be requested to obtain additional licensure in othergeographic areas
- Experience in gerontology, cardiology, internalmedicine, or endocrinology
- Home Health care or home visitexperience
- Ability to travel up to 5% within licensed states ifrequested for additional incentives (with notice, based on businessneeds) Pursuant to the San Francisco Fair Chance Ordinance, we willconsider for employment qualified applicants with arrest andconviction records
PLEASE NOTE
Employees must be in an activeregular status. Employees must remain in role for a minimum of 12months from the date of hire /rehire/transfer. If an employeeleaves Home and Community, the student loan repayments will cease.
The employee must remain in an Advanced Practice Clinician orPhysician role within Home and Community for 36 months to receivethe full benefit of the student loan repayments.

The salary range for this role is $88,000 to$173,200 annually. Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
