Aviator Medical Group PA is actively recruiting Nurse Practitioners with an active license in Texas or Florida. About the Role You’ll be the clinical engine behind our patient onboarding and care navigation model. You’ll conduct virtual initiating visits with Medicare beneficiaries, identify needs related to healthcare navigation access and the Social Determinants of Health, and enable our care team to deliver impactful Principle Illness Navigation (PIN) services. You’ll also help shape clinical policies, advise on care plan structure, and serve as a key thought partner as we scale our model. We’re looking for a clinician who is thoughtful, patient-centered, and energized by building a high-integrity, virtual-first care model from the ground up. In this role, you will: Conduct initiating visits via Telehealth with Medicare beneficiaries to assess medical needs, identify health care navigation-related barriers, and initiate PIN services. Oversee patient care plans in partnership with healthcare advocates to holistically address social determinants of health, i.e. food resources, transportation access, and support at home. Learn and develop our systems, tools, technology, partners, and expectations, while also providing your unique clinical expertise in every interaction. Qualifications and skills
Active State License:
Hold an active Nurse Practitioner license in Texas, with Board Certification in FNP, AGPCNP, AGACNP, or a related field. Strong documentation habits and comfort working within EHR systems. Interest in supporting underserved or medically complex older adults, including those navigating food insecurity, housing instability, or limited caregiver support. Flexible and pragmatic – able to balance clinical excellence with startup urgency.
