Nurse Practitioner – Medical Practice – Vascular Diseases, Diabetes & Pain Management RSC Healthcare Woodbridge, NJ 07095 Nurse Practitioner Job Description Seeking highly skilled and motivated Nurse Practitioners (NP) to join a dynamic team. This unique role involves travel to multiple clinic locations, providing essential care and support across our network. The ideal candidate will have experience in managing vascular diseases, musculoskeletal pain, and diabetes, and will be responsible for conducting event screenings and providing holistic care to our patients. This role requires both a strong clinical background and excellent patient management skills. Position location will be in Central New Jersey- rotating among 3 offices: Woodbridge, Marlboro, Matawan and doing some events in these areas

Key Responsibilities:

Perform comprehensive assessments, physical exams, and diagnostic evaluations for patients with vascular diseases, musculoskeletal pain, diabetes etc. Develop and implement individualized treatment plans, including prescribing medications and coordinating care with other healthcare providers. Conduct health screenings at events and community outreach programs to promote wellness and early detection of potential health issues (with sleep apnea, functional medicine etc.). Educate patients on managing their conditions, including lifestyle modifications, medication management, and preventive care strategies. Collaborate with physicians and other healthcare professionals to ensure coordinated and effective patient care. Document patient interactions, treatment plans, and progress in electronic medical records. Stay current with medical advancements and best practices in wellness, pain management, and chronic disease management. Travel to locations and events as needed.

Qualifications:

Master’s degree in Nursing (MSN) from an accredited institution. Active Nurse Practitioner (NP) certification from a recognized certifying body. Valid Registered Nurse (RN) license in New Jersey. (In New York as well is a plus) Minimum of 5 years of experience as an NP (with a focus on managing vascular diseases, musculoskeletal pain, and diabetes is a plus) Strong clinical skills with the ability to perform and interpret diagnostic tests. Experience with peripheral point injections (is a plus) Excellent communication and interpersonal skills, with a compassionate approach to patient care. Ability to work independently and as part of a multidisciplinary team. Experience in conducting health screenings and community outreach is a plus.
