Full Job Description General Overview:

NP provides comprehensive evaluations and treats the adult substance abuse population, while also coordinating mental health services.

Responsibilities:

Conduct evaluation and health assessment based on the patient’s reported substance abuse, medical, and mental health history. Order and review patient lab results to determine a treatment plan. Develop and evaluate a prioritized and mutually acceptable treatment plan according to the patient’s medical diagnosis and mental health needs. Responsible for the management of chronic and acute illnesses. Responsible for ordering and managing patient’s medications. Respond to medical emergencies within the facility. Assure patient’s medical history and physical examination are completed within twenty-four (24) hours of admission. Assure patients are referred to appropriate medical facilities when a higher level of care is needed. Collaborate patient care with other community providers. Provide admission, transfer, and discharge orders. Provide systematic follow-up of all implemented treatment plans. Provide patients with educational materials and information needed to make well informed choices. Collaborate with a multi-disciplinary team on executing patient-centered care. Responsible for documenting accurate assessments while remaining HIPAA compliant. Facilitate medical group education multiple times a week. Respond to same-day sick call appointments. Provide excellent patient care. Required to execute one on-call week per month. Perform any other duties as assigned by supervisor.

Qualifications :

• Master of Nurse Practitioner from an accredited educational institution.
• Valid and in good standing NYS Nurse Practitioner License.
• Valid and in good standing DEA license.
• Meet the qualifications of, and practice within the parameters as stated in Chapter 257 of the New York State Education Law, Article 139.
• Experience in substance abuse treatment preferred.
• Position requires constant mental alertness, attention to detail, and a high degree of accuracy.
• Strong problem-solving skills.
• Smart, driven, and exceptional work ethic.
• Must be able to follow oral and written instructions.
• Excellent organizational skills.
• Ability to work well in a group setting and autonomously.
• Excellent computer skills and ability to learn new systems.
