What You’ll Do:
Provide comprehensive care for short-term rehab and long-term residents Perform assessments, diagnose and treat chronic/acute conditions, and implement care plans Collaborate with nursing, therapy, social work, and rehab staff to enhance resident outcomes Maintain accurate EMR documentation, ensuring regulatory compliance Participate in quality initiatives and interdisciplinary case conferences
Who You Are:
Licensed NP in New York Experience in skilled nursing or rehab preferred Strong clinical judgment, communication, and collaboration skills Self-motivated and comfortable practicing independently in a team environment Why Join RiteMed at
