Local Contract About the Role Are you passionate aboutserving the geriatric population? We are seeking a compassionateand skilled Nurse Practitioner (NP) to deliver high-quality,patient-centered primary care services to residents in assistedliving and long-term care facilities. This is a meaningfulopportunity for providers who are dedicated to improving the healthand quality of life for older adults in a supportive, team-basedenvironment. Key Responsibilities

• Conduct comprehensive healthassessments, including physical exams, medication reconciliation,and review of medical histories.
• Develop and implementindividualized care plans in collaboration with interdisciplinaryteams.
• Manage chronic and acute health conditions with timelyinterventions and appropriate referrals.
• Order and interpretdiagnostic tests (labs, imaging, etc.) to guide clinical decisions
• Prescribe medications and treatments under the state’s scope ofpractice and facility policy.
• Initiate and participate in chroniccare management programs.
• Educate residents and families onhealth maintenance, disease prevention, and self-care.
• Participate in quality improvement efforts and interdisciplinarycare meetings.
• Maintain clear, accurate documentation in theelectronic medical record (EMR) system. Qualifications
• Master’sDegree in Nursing from an accredited Nurse Practitioner program.
• Active and unrestricted New York State NP license.
• Valid DEAregistration for prescribing.
• Eligible for reimbursement throughMedicare and Medicaid.
