Nurse Practitioner Revive Care Carmel Hamlet, NY Revive Care is a mobile wound physician group looking to expand our team, and currently seeking a few Nurse Practitioners to join in our mission of providing exceptional wound care services. This territory covers Putnam county, NY In this role, you will be responsible for on-site evaluations of patients’ wounds, collaborating with our mobile healthcare team, and developing individualized treatment plans. No prior experience is required, as we offer comprehensive training to ensure you are well-prepared to positively impact the lives of our patients.

Responsibilities:

Conduct in-home wound evaluations Collaborate with doctors and nurses to develop patient-specific treatment plans Provide compassionate and patient-centered care Maintain accurate and detailed medical records Participate in ongoing training and professional development

Qualifications:
