Duties and Responsibilities Essential Functions:
Obtains/records initial patient histories, assesses current health status, preform physical exams, identifies problems/needs and designs comprehensive management plans for patients. Assesses current health status, real and/or potential health problems and identifies expected outcomes in response to management plans, reviewing data with the physician as necessary. Requests appropriate diagnostic and screening procedures, interpreting the results. Writes prescriptions for required medication and/or treatment to meet management plan as allowed by State Regulations and protocols. Coordinates continued care for individual patients and provides follow up evaluation of patients’ response to management plans. Provides emergency care as indicated and supported by protocols, training and experience. Acts as a resource, educator and consultant to staff. Develops and evaluates standards of care and protocols as necessary. Provides patient and family education, support and follow up as needed. Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc.

Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s). Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and “error proof” systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and workshops. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. Attends meetings as required. Exposure Qualifications Minimum Education:
Masters Degree Nurse Practitioner Required or Bachelors Degree Physician Assistant Required Masters Degree Physician Assistant Preferred Work Experience:
1 year
Required Licenses:
Certified Registered Nurse Practitioner Upon Hire Required or Licensed Physician Assistant Upon Hire Required and Basic Life Support Upon Hire Required or Advanced Cardiac Life Support Upon Hire Required Courses and Training:
BLS and/or ACLS as appropriate for the clinical role. Upon Hire Required and Nurse Practitioner
- board certified by American Nurses Credentialing Center Commission on Certification Upon Hire Required and Nurse Practitioner
- Completion of an accredited program for nurse practitioners Upon Hire Required or Physician Assistant
