We provide high-quality, person-centered care to clients in their homes, ensuring they maintain dignity, independence, and comfort. Key Responsibilities Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding. Provide light housekeeping, meal preparation, and medication reminders. Monitor and report any changes in the client’s physical and emotional condition. Assist with mobility and transfers using proper techniques and equipment. Provide companionship and emotional support to enhance the client’s quality of life. Document care provided and communicate effectively with supervisor and care team members. Follow individualized care plans and adhere to Ann’s Dream Healthcare policies to ensure safe, respectful, and high-quality care Qualifications High School Diploma or GED. Active Certfied Nursing Assistant (CNA) certification required. Two years previous experience in home health, personal care, or facility care. Experience working with children with Autism, ADHD or a Developmental Delay. Must be compassionate, reliable, and professional. Ability to work independently and follow detailed care plans. Strong communication and organizational skills. Reliable transportation to client homes.
