Heart Care Transition Clinic Position:
Nurse Navigator Advanced Heart Failure Clinic What Youll Do:

As a Nurse Navigator in the Advanced Heart Failure Clinic, youll play a pivotal role in guiding patients and their families through complex care journeys with compassion and clarity. Youll coordinate care across the continuum from diagnosis through recovery using a multidisciplinary approach to ensure each patient receives timely, effective, and personalized care.

Key Responsibilities Include:

Serve as a clinical liaison among patients, families, staff, and referring providers
Provide in-person and phone-based education, counseling, and support
Identify patient and staff educational needs and create impactful learning tools
Coordinate access to community resources and follow-up services
Help eliminate barriers to care and promote continuity, quality, and satisfaction
Function as a clinician, advocate, facilitator, consultant, and educator

Qualifications:

Bachelors Degree in Nursing Required
Registered Nurse License State of Florida Required
BLS Certification Required
Minimum 3 years of experience in Cardiology Required
Proficiency in Microsoft Word, Excel, PowerPoint, and Windows-based systems We are dedicated to providing a caring and compassionate environment for our patients and our staff.
