Position:

As part of a person centered care model, this role willbe responsible for assisting in the implementation and oversight ofall aspects of case management. Using best practice concepts, thisrole will assist the clinical team in care planning, appropriateutilization of services and coordination of care. The primary focusof this remains on providing person centered care efficiently andensure patient satisfaction. This role will collaborate with allteam members to achieve exceptional clinical, quality and financialoutcomes. In general, most of the time will be spent in thefollowing activities:

• Reviews clinical information and assessmentdata and makes recommendations for efficient and effectiveutilization of services to achieve positive patient outcomes andestablished organizational metrics.
• Analyzes data, investigatesoutliers and reports appropriately
• Participates in daily andweekly case conferences with the clinical teams
• Promotes acooperative, cohesive group process dedicated to provision ofquality patient care with achievement of best possible patientoutcomes; collaborates with peers across regions
• Participates inPerformance Improvement activities within the Agency
• Responsiblefor the quality, clinical, financial and patient satisfactionoutcomes
• Responsible for case management activities including butnot limited to authorization of services, identification ofphysicians, compliance with the plan of care.
• Maintains knowledgeof and compliance with Federal, State and accreditation regulationsas well as Agency policies and procedures
• Maintains utilizationstatistics in line with national best practice benchmarks andoptimizes clinical
• outcome scores as evidenced by Home HealthCompare and its equivalents
• Run Daily clinical huddles,participate in IDTs, Lean Daily Management, and participates indaily SBAR with OASIS and revisit nurses
• Coaches and evaluatesLicensed Professional Nurses and is responsible for completion ofpersonnel evaluations according to Agency policy.

QualificationsLicensure:

Registered Nurse with a license to practice in the

Stateof CT Education:

Graduate of an Associate’s or diploma program innursing; graduate of NLN approved BSN program, preferred

Experience:

One year nursing experience or one year nursingexperience in a specialty; medical/surgery or home care, preferred.
