Develop and implement Beacon therapy plans for patients with cancer, utilizing specialized knowledge and expertise to optimize patient outcomes.
Collaborate with a multidisciplinary team to assess patients’ medication regimens and make recommendations for changes or adjustments as needed.
Conduct thorough medication reviews and provide patient education on proper medication usage, potential side effects, and drug interactions.
Monitor and evaluate the effectiveness of medication therapy and make necessary interventions to ensure optimal patient outcomes.
Serve as a resource for healthcare providers and other staff members regarding medication therapy for patients with cancer.
Stay current with the latest developments and research in the field of oncology and incorporate new information into practice.
Participate in quality improvement initiatives to enhance patient care and safety.
Maintain accurate and detailed patient records, including medication histories, assessments, and interventions.
Provide clinical guidance and support to pharmacy staff, including technicians and students.
Collaborate with other healthcare professionals, such as physicians and nurses, to ensure coordinated and comprehensive care for patients with cancer.
Maintain compliance with all relevant laws, regulations, and policies related to pharmacy practice.
Participate in professional development activities, such as continuing education, to maintain and enhance clinical skills and knowledge

Additional Job Description Minimum Training and Education:

Doctor of Pharmacy or a Bachelor’s degree in pharmacy. Completion of a pharmacy residency preferred.

Required Licensure, Certifications, Registrations:

Licensed as a Registered Pharmacist by the South Carolina Board of Pharmacy.
