Orthopedic At-Home Physical Therapist JAG Physical Therapy isseeking to hire a skilled Physical Therapist specializing inorthopedic rehabilitation in the home setting for patientsrecovering from total hip and knee replacement surgeries and otherorthopedic related conditions. Our mission is to help patientsregain mobility, strength, function and confidence in the comfortof their own homes. This approach emphasizes not only physicalrecovery, but also the emotional and social benefits of re-engagingwith the community. We are looking for a compassionate andmotivated physical therapist to join our team and make a differencein the lives of those recovering and returning to their prior levelof function. Our physical therapists will be required to evaluateand treat the patients for the duration of their plan of care intheir home setting with the goal of transitioning into one of ourJAG outpatient orthopedic locations for continuity of care. WhatYou’ll Need:
- Bachelor’s or Master’s Degree required, Doctoratedegree preferred
- Licensed Physical Therapist in the statepracticing in
- Completed state regulated CEU requirement inappropriate state (NY, NJ)
- Valid CPR License
- Experience/knowledge with an EMR software is preferred
- Excellentoral, written communication
- Strong clinical decision-makingskills
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Works as a team member and individuallywith minimal supervision
- Maintains a positive, professionaldemeanor at all times
What You’ll Do:
- Educate patient on safe bedmobility, transfers and ambulation in consideration of postoperative precautions
- Transfer, ambulate and perform stairnegotiation with patients to determine they are safe for dischargeto home
- Collects and records patient care data that followsformat and guidelines of the patient care service, regulatoryagencies and third-party payers which reflects an understanding ofreimbursement systems and their impact on patient care.
- Thisincludes, but is not limited to, initial evaluation, daily notes,reassessments/progress reports, discharge plans and communicationwith members of the team.
- Additionally, completes daily billingand follows department guidelines for productivity.
- Prepares forthe evaluation in accordance with department policies/proceduresand standards of practice including obtaining patient history,exhibiting knowledge of patient diagnoses and researchingunfamiliar diagnoses.
- Selects and administers tests and measures.
- Additionally, synthesizes evaluation results to formulate thediagnosis, goals, prognosis and plan of care.
- Establishestreatment plan based on evaluation results, goals and bestpractice. Collaborates with interdisciplinary team, patients andpatient’s support system. Assesses and re-assesses need formodifications to plan of care and goals.
- Provideseducation/training to patient and/or patient support system
- Functional Training-This includes but is not limited to ADLs/IADLs,therapeutic activities (e.g. transfers, bed mobility),rehabilitation equipment prescription and training and communityreintegration
- Responsible for treatment, plan of care, exerciseprescription for patient caseload
- Provides interventions topatients that are safe, effective, and in compliance with JAG
- Provide data, note writing using Prompt EMR for all treatmentsprovided
- Attends annual JAG Billing and Coding seminar
- Communicate with Manager/Patient Access team for schedulingevaluations once demographics are received
