Location 1: 3 rooms 2 CRNA + 1 ANX MD Supervision model Monday-Friday, 7a 5p
Daily Volume:
30 patients Location 2: 4 rooms 4 CRNA + 1 ANX MD Supervision model Monday-Friday, 7:30a 3:30p
Daily Volume:
30 patients Location 3: 2 rooms, MAC 2 CRNA + 1 ANX MD Supervision model Monday-Friday, 7a-5p
Daily Volume:
12 patients Location 4: 3 rooms (1 general
