We are seeking a dedicated and detail-oriented MDS Coordinator to join our skilled nursing team. This role is responsible for coordinating and completing the MDS process, ensuring accurate documentation, and overseeing the restorative nursing program. The ideal candidate will have strong clinical judgment, excellent communication skills, and a passion for improving resident outcomes.
Essential Responsibilities:
Coordinate, conduct, and complete MDS assessments and resident care plans using the electronic medical record system. Ensure accuracy and completeness of all medical records. Evaluate residents’ clinical needs, nutritional status, and rehabilitation potential to determine Medicare eligibility. Monitor timely and compliant completion of MDS assessments per HCFA and Medicare guidelines. Ensure electronic submission of MDS data in accordance with state regulations. Participate in resident and family care plan conferences. Facilitate effective communication across shifts and departments regarding medical documentation. Oversee the community’s restorative program and assign appropriate restorative nursing interventions. Manage Restorative Aides, including hiring, training, performance evaluation, and development.
Additional Responsibilities:
Provide direct nursing care as needed to support staffing levels. Respond to emergency calls and changes in resident conditions. Ensure timely physician visits and notify physicians/families of resident status changes. Promote rehabilitative techniques to support resident independence. Assist the DON in developing and implementing medical record policies and procedures. Attend required in-service training and educational sessions. Perform other duties as assigned by the supervisor.
Qualifications:
Current RN or LPN license in FL Experience in long-term care or skilled nursing Prior experience with MDS 3.0 and restorative nursing programs preferred Strong knowledge of Medicare/Medicaid regulations Excellent organizational, leadership, and communication skills Proficiency with electronic medical records (EMR) systems
