Nurse Practitioner – Preventive Cardiology | Naples, FL & Surrounding Areas Health Monitor 1 Naples, FL 34112 We’re hiring a Nurse Practitioner to help us deliver high-quality, preventive care through in-person visits across the Naples region. If you’re passionate about cardiology, chronic care, and patient-centered service, this is your opportunity to make a meaningful impact, one visit at a time. What You’ll Do Conduct in-person visits focused on cardiovascular prevention and chronic disease management in Assisted Living Facilities (ALFs) and Skilled Nursing Facilities (SNFs) Monitor patients’ cardiac conditions daily using remote monitoring technology Collaborate with primary care providers, specialists, and our internal clinical team Document clinical notes and care plans accurately and professionally Educate patients, families, and facility staff on heart health, prevention, and wellness Travel to different care locations across Naples and nearby areas Contribute to a positive, excellence-driven care environment You’re a
Great Fit If You:
Hold an active Nurse Practitioner license in the state of Florida Have experience or strong interest in cardiology, chronic disease care, or long-term care settings Are familiar with Assisted Living or Skilled Nursing environments (preferred) Are confident working independently, yet value teamwork and clear communication Are comfortable with technology and remote monitoring tools Are organized, reliable, and detail-oriented Have excellent documentation and communication skills Are authorized to work in the U.S. Are willing to travel locally between care sites in the Naples area
