Responsibilities:

The CareManagement Nurse, MDS Nurse works the RAI process and conductsassessments and care plan coordination for those residentsassigned. Some responsibilities of the MDS nurse include: Completesthe MDS, CAA’s and care plans within regulated time frames.

Assesses resident through physical assessment, interview and chartreview. Discusses resident care needs with care givers, includingphysician, nursing, social services, therapy, dietary, and activitystaff. Reviews information from hospital, consults and outsideagencies and uses such information in the completion of theassessment and care planning. Coordinates, identifies, and/orinitiates significant change MDS’ Is prepared to conductutilization review meetings maintaining MDS assessments perMedicare schedule and maintains skilled board for monitoring ofMedicare days and PDPM utilization in the absence of the CareManagement Coordinator Remains current with the AmericanAssociation of Nursing Assessment Coordinators (AANAC)requirements.

Qualifications:

Registered Nurse, RN or LicensedPractical Nurse with experience may be considered AANCcertification a plus. RAC-CT Knowledge of the Resident AssessmentInstrument (RAI) process, including the principles the ProspectivePayment Process (PPS) strongly preferred.
