Cardinal Hospice is adding an R.N. Case Manager to our team. The position is for someone who is looking for a warm, friendly environment, work/life balance with a M-F schedule, rotating on-call with the other nursing staff members. Principle Duties:
As a Hospice Registered Nurse Case Manager, you will play a critical role in providing holistic care to our patients and their families.
Your responsibilities will include:
Conducting initial assessments and developing personalized care plans. Administering medications and providing symptom management. Completes all mandatory documentation in a timely (determined) fashion as established by applicable regulations. Collaborating with a multidisciplinary team of healthcare professionals. Offering emotional support and education to patients and families. Ensuring that patients receive the highest quality of care during their end-of-life journey.
Qualifications:
Current Registered Nurse license in the state of Louisiana Two years of experience as a Registered Nurse in a clinical care setting required. Hospice experience preferred but not required. Strong communication and interpersonal skills Compassionate and empathetic nature. Ability to work collaboratively in a team environment. Current proof of CPR Certification Valid Drivers’ License, auto registration and insurance.
