  • One year of recent experience as aRegistered Nurse, including a minimum of six months in a HomeHealth setting as an RN/n/t
  • Proficient in OASIS documentation/n/t
  • Valid RN license in compliance with state regulations/n/t
  • BLScertification (AHA/ARC)/n /n/n
Preferred Qualifications:

  • Home Health Nursing Certification (RN-BC)/n/t

Summary:

/n/n A Home Health Registered Nurse (RN) deliversdirect nursing care to patients in their homes, ensuring comfort,stability, and effective management of their medical conditions.

Home Health RNs assess patient conditions, develop care plans,administer treatments, and educate patients and caregivers ondisease management. /n/n

Essential Work Functions:

  • Assesspatient conditions and develop individualized care plans based onmedical needs and physician directives/n/t
  • Administermedications, treatments, and wound care while monitoring patientresponses/n/t
  • Provide patient and caregiver education onmedication management, disease progression, and self-caretechniques/n/t
  • Monitor for changes in patient status, reportingconcerns and adjusting care plans as needed/n/t
  • Coordinate withphysicians, therapists, and other healthcare professionals toensure seamless care transitions/n/t
  • Document patient care,interventions, and progress accurately and in compliance withregulatory standards/n/t
  • Ensure compliance with infectioncontrol, safety, and home care best practices to protect patientsand caregivers/n/t
  • Support families and caregivers, offeringguidance and emotional support throughout the care process/n/t
  • Maintain communication with case managers and interdisciplinaryteams to provide coordinated patient care/n/t
  • Strong ability tocomplete charting and documentation requirements while meetingvisit and productivity goals/n/t
  • Adhere to infection control,safety, and healthcare regulations/n/t
  • Perform other duties asassigned within the scope of practice/n /n/n
