JobSummary The Nurse Coordinator will be functioning with the contextof the framework for professional nursing practice. The TransplantNurse Coordinator provides and coordinates the multidisciplinaryclinical care of potential and actual transplant candidates andrecipients. Coordinates the clinical management of pre orpost-transplant patients. Manages data for transplant patients.
Plans, develops, implements and evaluates educational programs andpublications designed to meet the needs of nurses, medicalpersonnel, patients, families and the community. Reports to theManager.
Responsibilities may include:
- Conducts thepre-transplant interview including health, history, andpre-transplant patient teaching, coordinates the pre-transplantevaluation, communicates with the referring physician, presentspatient to the transplant team and places the patient on the activetransplant list to ensure appropriateness of the patient fortransplantation.
- Participates in the Transplant Health Care Teamin establishing and accomplishing goals, functions as a changeagent by consulting and communicating with care-givers acrossclinical areas related to improving the clinical outcomes and costeffectiveness of transplantation patient car
- Identifieseducational needs of patients, families, populations, health careproviders and students. Develops and/or guides development ofprograms, protocols and policies to meet those needs.
- Attendstransplant rounds, conducts pre-transplant visits to potentialinpatients and conducts follow-up visits to patients after theirtransplantation to assess and evaluate the needs of the patient
- Participates in research studies and interprets, disseminates andutilizes findings in practice to improve quality, cost and outcomesof patient care(e.g. standards, program development, changes incare). Mentors and coaches staff in conducting research andutilizing findings
- Represents the organization at local, regionaland national levels through speaking, presenting and publishing tohelp to identify the organization as a center of excellence
- Demonstrates competencies including knowledge and behaviors, tomeet age-specific patient needs for the heart transplant populationor for the management of cadaver organ donors
- Models qualitypatient/customer service by: demonstrating a willingness to serveall customers at all times, maintaining an awareness ofpatient/customer needs and responding to those needs; maintainingconfidentiality of all patient information and functioning in acollaborative manner with other care-givers to ensure qualitypatient care
- Contributes to care improvement by: coaching,developing and mentoring staff, participating inperformance/quality improvement opportunities and research, andthrough coordinating development and annual review of standards ofcare with other Health Care Team members. Qualifications
- CurrentConnecticut registered Nurse License. (BSN Required)
- 2-4 yearsrecent clinical acute care experience.
- Experience intransplantation preferred.
