Required Qualifications:

• One year of recent experience as a Registered Nurse, including aminimum of six months in a Home Health setting as an RN
• Proficient in OASIS documentation
• Valid RN license in compliancewith state regulations
• BLS certification (AHA/ARC)

PreferredQualifications:

• Home Health Nursing Certification (RN-BC)
• Othercertifications and licenses may be required for this position

Summary:

A Home Health Registered Nurse (RN) delivers directnursing care to patients in their homes, ensuring comfort,stability, and effective management of their medical conditions.

Home Health RNs assess patient conditions, develop care plans,administer treatments, and educate patients and caregivers ondisease management.

Essential Work Functions:

• Assess patientconditions and develop individualized care plans based on medicalneeds and physician directives
• Administer medications,treatments, and wound care while monitoring patient responses
• Provide patient and caregiver education on medication management,disease progression, and self-care techniques
• Monitor for changesin patient status, reporting concerns and adjusting care plans asneeded
• Coordinate with physicians, therapists, and otherhealthcare professionals to ensure seamless care transitions
• Document patient care, interventions, and progress accurately andin compliance with regulatory standards
• Ensure compliance withinfection control, safety, and home care best practices to protectpatients and caregivers
• Support families and caregivers, offeringguidance and emotional support throughout the care process
• Maintain communication with case managers and interdisciplinaryteams to provide coordinated patient care
• Strong ability tocomplete charting and documentation requirements while meetingvisit and productivity goals
• Adhere to infection control, safety,and healthcare regulations
• Perform other duties as assignedwithin the scope of practice

• The ability to read, write, & communicate in the Englishlanguage is required
