Vincent’s Medical Center is the first hospital in Fairfield Countyto be integrated into the Hartford HealthCare network, and part ofone of the largest providers of mental health services at severallocations in Connecticut.
Job Summary:
Provides direct patient careto behavioral health patients in accordance with the establishedpolicies and procedures of the facility.
Responsibilities:
Performsinitial and on-going bio-psychosocial assessments according toaccepted standards of nursing practice. Develops a nursing plan ofcare consistent with the master treatment plan. Participates intreatment team conferences to assist in planning and revisinggoals, objectives and interventions appropriate to the age-relatedand problem-specific needs of each patient. Implements nursing planof care for assigned patients and conducts and/or co- leads grouptherapy sessions for patients Evaluates patient’s response tointerventions and revises nursing plan of care as needed.
Collaborates with the treatment team to revise goals, objectivesand interventions appropriate to the changes in patient status.
Performs appropriate assessments of patients per hospital policyand identifies physical, psychosocial, cultural spiritual andage-related needs in order to provide individualized care of thepatient. Develops and documents a comprehensive plan of nursingcare in collaboration with the patient and family to meet thephysical, psychosocial, spiritual, cultural and age-related needsof patients in order to facilitate the delivery of care. Providesdirect patient care according to physician’s order and appropriateprotocols as evidenced by documentation in patient’s medical recordand direct supervisor or peer observation. Evaluates and recognizeschanges in patients’ physical and psychosocial status and revisesthe plan of care as necessary. Delegates aspects of care to othernursing personnel, consistent with their level of education andexpertise in order to meet patients’ needs and to maximize staffperformance. Demonstrates the knowledge and skills necessary toprovide care appropriate to behavioral health patients and tointeract with the interdisciplinary team in order to achieve thedesired outcomes for patients. Other Duties as assigned ReflectsANA Scope and Standards of Nursing Practice (2015), ANA Code ofEthics (2015), and ANA Social Policy Statement (2015).Qualifications Licensed Registered Nurse credentialed from theConnecticut Department of Public Health obtained prior to hire dateor job transfer date required Certified BLS Provider credentialedfrom the American Heart Association (AHA) or American Red Crossobtained within 2 Months (60 days) of hire date or job transferdate Associate’s Degree required, Bachelor Degree preferredExperience in an acute care psychiatric setting is preferred.
