Monday-Friday, either8:00am-4:30p or 8:30a-5:00p depending on the clinic volume.

Rotating Saturdays (non-patient care shift)

RESPONSIBILITIES

• Patient Care & Service.

Promote and restorepatients’ health by completing the nursing process; collaboratingwith physicians and multidisciplinary team members; performingvarious treatment procedures; providing physical, educational andemotional support to patients, friends and families; supervisingassigned team members

• Planning & Communication.

Develop anddocument individualized care plans customized for each patient’sunique needs, with support from the interdisciplinary health teamas needed; maintain effective communication to convey patienthealth status, treatment plans and progress

• Electronic HealthRecord (EHR) Management.

Demonstrate proficient use of an EHR

• including accurate patient and provider documentation andcommunication
• Compliance.

Adhere to required department and systemprotocols, regulations (local, state, federal) and educationrequirements

Required Qualifications:

• Registered Nurse license inNew York State.

Valid NYS Driver’s License PreferredQualifications:

• 1 year of nursing experienced preferred.
• Priorhome health, clinical and direct patient care experience preferred.
