Full Time SUMMARY:
The R.N. IV maintains quality patient care through consistent standards of nursing practice in collaboration with the multidisciplinary team.
Utilizes the nursing process:
assess, diagnosis, plan, implement and evaluate to execute appropriate individual patient care. Responsible for orientation, training/precepting, and mentoring of other personnel on the unit. Provides education to the personnel on the unit, and connects Rochester Regional Health to the nursing profession through research, publications and presentation. The R.N. is instrumental in providing an exceptional experience for patient and family.
RESPONSIBILITIES
Provides high-quality, patient-centered, clinical care:
Assessment:
Performs initial and ongoing assessment of the patient, utilizing appropriate data (i.e. physical, psychological, educational and risk-screen data appropriate for the chronological / developmental age of the patient). Demonstrates exceptional clinical judgment and advanced assessment skills in responding to priority patient needs. Communicates and documents assessment of patient with the interdisciplinary team. Utilizes all available resources in order to meet the needs of the patient and family.
Planning:
Develops and documents plan of care, treatment and services which is individualized and appropriate for the patient and family needs, strengths, limitations and goals. Includes the interdisciplinary health team in healthcare decision-making Demonstrates exceptional and effective communication skills with the patient/family and interdisciplinary team.
Implementation:
Provides exceptional care, treatment and services in collaboration with interdisciplinary health team. Maintains an appropriate nurse-patient relationship and provides an exceptional experience for the patient and family. Complies with organizational policies, procedures and protocols including policies governing safety, medication administration and documentation. Initiates and documents individualized education plan for the patient and family based on identified learning and uses the teach back method for evaluation of learning.
Evaluations:
Evaluates, communicates and documents patient responses to care. Revises plan of care based on ongoing evaluations and updates documentation appropriately.
Education:
Ensures the patient receives / understands education / training specific to their needs and abilities using the teach back method and documents appropriately. Demonstrates compassion, leadership and professionalism: Assumes responsibility and accountability on the unit / patient care team. Demonstrates the ability to work collaboratively with the interdisciplinary team. Demonstrates understanding of variable staffing patterns and manages to productivity targets. Actively participates on a unit based or hospital wide committee. Actively participates in at least one performance improvement project that is unit-based or hospital-based annually. Serves as a clinical expert in area of specialty. Demonstrates knowledge of legal aspects of nursing as evidenced by documentation and professional behaviors. Delegates appropriate responsibilities to assigned nurses and ancillary personnel. Demonstrates ability to manage job related stressors and constructive feedback during intra/interdepartmental relations. Demonstrates flexibility in meeting staffing needs throughout the organization. Demonstrates clinical competency in area of specialty and participates in providing clinical competency education to other members of the team. Participates in problem-solving activities at the unit level by identifying the problem and collaboratively working toward resolution. Provides input regarding protocols, policies and procedures to immediate supervisor. Performance improvement / evidence based practice / research Demonstrates knowledge of evidence-based practice within specialty area and identifies opportunities for performance improvement. Participates and utilizes the performance improvement process to improve clinical practice. Completes a minimum of one approved unit or divisional performance improvement project annually. Completes at least one clinical research utilization (CRU) form for submission to Nursing Research Department utilizing a research article from the last 5 years from research performed in the United States. Compliance Annually meets the minimum requirements of the RN IV according to the clinical ladder policy Completes mandatory in-services and health screens in timely manner. Demonstrates competency in area of specialty as well as serves as a clinical expert for skills competency in area of practice. Completes Preceptor training. Maintains Basic Life Support (BLS) certification.
Training / Precepting
Provides training / precepting for new team members on the unit. Assumes role of teacher for nursing students and other students shadowing on the unit. Provides mentoring to others on the unit to ensure the best clinical outcome is reached. Acts as a resource for other team members for both clinical and non-clinical matters. Identifies and assists other team members in identifying learning needs and communicates to appropriate resource. Demonstrates competency in required skills as well as EMR related skills and knowledge.
REQUIRED QUALIFICATIONS
Bachelor’s Degree in Nursing required. Minimum of 6 months experience as a registered nurse Registered Nurse license in New York State. Basic Life Support (BLS) certification.
