The Supportive Care RN coordinates care and providesinitial ongoing nursing assessment of the patient and family needsand coordination of the patient’s plan of care withinterdisciplinary team members and the Supportive Care attendingphysician.
Primary Responsibilities:
- Conducts medical home visitson established Supportive Care patients performing a hands-on examat each visit
- Conducts urgent/acute visits on establishedSupportive Care patients with the goal of keeping the patient outof the hospital in order to receive the best medical care at home
- Provides ongoing assessments of the impact of life-limiting and/orchronic illness(es) on the patient’s physical, functional,psychosocial and environmental needs
- Assess for caregiverburnout, and provide community resources when appropriate
- Implement the individualized plan of care and recommend revisionsto the plan as necessary
- Ability to perform procedures, toinclude, but not limited to:
- a. Wound care and dressing changes
- b. Phlebotomy
- c. Injection (B12, vaccines)
- Ability to conductan advanced care discussion with a patient and their family andproperly document their wishes in the electronic medical record
- Consults and educates the patient/family and other caregiversregarding the disease process, pain and symptom management, end oflife care and processes for dealing with issues of ethical concern
- Initiates appropriate preventive and rehabilitative nursingprocedures when appropriate
- Ensure continual assessment ofpatient and family needs from admission to Supportive Carethroughout the course of care
- Provides ongoing evaluation of thepatient and family/caregiver response to care, and recommendsalteration of the plan when necessary
- Attends theinterdisciplinary meeting and is a crucial and vocal member of theteam
- Solves problems by gathering and/or reviewing facts andselecting the best solution from identified alternatives. Decisionmaking is usually based on prior practice or policy, with someinterpretation.
- Applies individual reasoning to the solution of aproblem devising or modifying processes and writing procedures
- Conducts telephonic nursing follow up and case management whennecessary
- Serves as a resource or consultant for LVN
- Attendseducational offerings to keep abreast of palliative care andcomplies with licensing requirements
- Establishes a trustingrelationship with identified patients, caregivers, clinic staffmembers and physicians
- Collaborates with the providers torecommend policies, procedures and standards which affect the careof the Supportive Care Patient
- Exhibits professionalism and iscourteous with all patients, physicians and co-workers
- Informsthe provider of patient’s needs and outcomes of interventions asper standards
- Follows Supportive Care providers’ orders regardingthe scope and frequency of services needed based on acuity andpatient/family needs
- Coordinates all patient/family services andprioritization of needs with the members of the interdisciplinaryteam
- Documents in the electronic health record progress towardestablished goals as per standards
- Uses the case managementapproach and refers to other services as needed
- Maintains apatient case load, daily visits and point of care documentationlevels as per standards
- Participates with the on-call rotationschedule and weekends as assigned

RequiredQualifications:
- Registered Nurse with a current license topractice in the state of employment
- Current BLS certification ormust obtain certification within 30 days of employment hire date
- 5 years of experience in a physician’s office, clinical, hospice orhospital setting
- Demonstrated experience of excellent nursingskills
- Experience related to advanced care planning anddiscussions with patients regarding end-of-life wishes
- Proficientknowledge of palliative care and end-of-life symptom management
- Proficient computer skills, including Microsoft Word, Excel,Access, Outlook, and Teams
- Proven excellent verbal and writtenskills
- Proven ability to interact productively with individualsand with multidisciplinary teams
- Proven excellent organizationaland prioritization skills
- Reliable transportation for dailytravel to various locations as assigned
- Must have a validdriver’s license within the state of work Preferred Qualifications
- 2 years of hospice experience
- Hospice and Palliative CareNursing certification
- Experience with community-based nursing
- Knowledge of palliative and hospice medicine
- Bilingual(English/Spanish) language proficiency
Physical & MentalRequirements:
- Ability to lift up to 100 pounds
- Ability to sitfor extended periods of time
- Ability to use fine motor skills tooperate office equipment and/or machinery
- Ability to properlydrive and operate a company vehicle
- Ability to receive andcomprehend instructions verbally and/or in writing
- Ability to uselogical reasoning for simple and complex problem solving The salaryrange for this role is $59,500 to $116,600 annually based onfull-time employment.
Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
